في العلاج قال بندي metyrapone (11-B- hydroxylase inhibitor )

1. بيعمل block of ACTH on adrenal gland
2. improve clinical condition before surgery (transsphenoidal surgery for Cushing's disease or adrenalectomy).
3. In para neoplastic syndrom

ليه لما اعمل surgical removal of adrenal tumer لازم اعوض ب cortisone ؟؟؟

عشان ميدخلش في adrenal crisis * ..؛ عشان كمان نمنع حدوث nelson syndrome* ( شوف هو المشكله كانت انه في ورم ف الغده اانخاميه وعماله تطلع ACTH كتير وبالتالي الغده الكظريه تطلع كورتيزون كتير فبيعمل FB للنخاميه انها تهدا والورم يهدى شويه من تطليع ACTH تخيل بقا جينا شيلنا الغده الكظريه في الجنبين، دلوقتي مفيش كورتيزون يعمل FB inhibition on pituitary tumor فيطلع ACTH كتير جدا و يعمل hyperpigmintation ودا في ٢٠-٨٠٪ من الحالات والورم يكبر Pituitary corticotroph adenoma)، مما يؤدي إلى:

· زيادة حجم الورم (Macroadenoma) ودا اللي هو nelson s وف الحاله دي بندي cortisone عشان نثبط الtumer شويه زي ما كنا ، هذا ما علمت والله أعلى وأعلم )و الحمد لله رب العالمين
