_🔴Patient safety:

prevention of injuries produced by medical care, detection of hazards proactively & implementation of changes to prevent these events

_🔴Classification of medical errors:

1-Active (person): are the events occurringimmediately before an incident or accident,types of active :slips,lapses,fixations

2-Latent (System) :

are problems lurking withinsystems, which under certain conditions willcontribute to an error occurring.

1-Adverse event

2-Near miss

_🔴Difference between Adverse event & Near miss :

*🔴Adverse event :

event that results in unintended harm tothe patient by an act of commission oromission rather than by the underlyingdisease or condition of the patient (اللي حصل حصل )

*🔴Near miss:

An error of commission or omission thatcould have harmed the patient, but seriousharm did not occur and was prevented fromcompletion through a planned or unplannedrecovery.
