๐ฃ Anatomy of the Scalp & Face โ Notes
๐ Scalp
๐ Definition: Hair-bearing part of the head.
๐ Boundaries:
- Anterior: Eyebrow โ Glabella โ Supraorbital margin
- Posterior: External occipital protuberance โ Superior nuchal line
- Lateral: Superior temporal line โ Zygomatic arch
๐ Layers (S.C.A.L.P):
- Skin โ with hair & sweat glands
- Connective tissue โ rich blood vessels ๐จ bleeding source
- Aponeurosis โ occipitofrontalis tendon
- Loose areolar tissue โ fat, allows free movement ๐จ danger space for infection
- Pericranium โ periosteum of skull
๐ Occipitofrontalis Muscle
๐ Two bellies connected by aponeurosis:
- Frontal belly โ elevates eyebrow, wrinkles forehead (surprise)
- Occipital belly โ pulls scalp backward, smooths forehead
๐ Nerve supply:
- Frontal belly โ Temporal branch of CN VII
- Occipital belly โ Posterior auricular branch of CN VII
๐ Nerve Supply of Scalp & Face
๐ Motor: Facial nerve (CN VII) โ all muscles of facial expression
๐ Sensory: Trigeminal nerve (CN V)
- V1 (ophthalmic): forehead, nose, medial eye
- V2 (maxillary): upper lip, zygomatic region
- V3 (mandibular): chin, lower lip, temporal region
๐ Exceptions:
- Angle of mandible โ Great auricular nerve (C2, C3)
- Posterior scalp โ Greater, Lesser, Third occipital nerves
๐ Muscles of Facial Expression
๐ Orbicularis oculi
- Orbital part โ strong closure (squinting)
- Palpebral part โ gentle closure (blinking)
- Lacrimal part โ aids tear flow
๐จ Origin & insertion: medial palpebral ligament โ same ligament
๐ Buccinator
- Prevents food pocketing between cheek & gums
- Helps blowing & whistling
- Supplied by buccal branch of CN VII
๐ Orbicularis oris
- Encircles mouth โ closes/open lips
- Actions: kissing, whistling
๐ Applied Anatomy โ Bellโs Palsy
๐ Cause: LMN lesion of Facial nerve (CN VII)
๐ Signs:
- Inability to close eye โ dryness, corneal damage
- Drooping mouth, deviation to healthy side
- Canโt whistle, blow, or retain saliva
๐ Development of the Face
๐ Formed around Stomodeum
๐ Processes:
- Frontonasal: nose, philtrum
- Maxillary (1st arch): upper lip (except philtrum), cheek, nasolacrimal duct, palate
- Mandibular (1st arch): lower lip & chin
๐จ Key Exam Questions
- Layer of scalp causing severe hemorrhage โ Connective tissue
- Function of loose areolar tissue โ allows free movement of scalp
- Motor supply to facial muscles โ Facial nerve (CN VII)
- Sensory supply to face/scalp โ Trigeminal nerve (CN V)
- Sensory over angle of mandible โ Great auricular nerve
- Action of occipitofrontalis โ surprise (frontal), smoothing (occipital)
- Key role of buccinator โ prevents food pocketing
- Orbital vs palpebral part of orbicularis oculi โ strong vs gentle closure
- Development: Maxillary & Mandibular โ First pharyngeal arch
