🧠 The Midbrain and Related Structures
🟣 1. General Anatomy
🖇 The Midbrain = part of the brainstem between the pons and diencephalon.
🖇 Divided by the Cerebral Aqueduct of Sylvius into:
- Tectum (dorsal) → lies above the aqueduct.
- Cerebral Peduncle (ventral) → below the aqueduct.
🟠 2. Main Parts of the Midbrain
📌 A. Tectum (Dorsal Part):
- Contains 4 Colliculi → 2 Superior + 2 Inferior (Corpora Quadrigemina).
- Centers for visual and auditory reflexes.
📌 B. Cerebral Peduncle (Ventral Part): subdivided into:
- Crus Cerebri (Basis Pedunculi):
- Contains descending tracts:
- Corticospinal (middle 3/5)
- Corticobulbar (medial)
- Corticopontine (lateral)
- Substantia Nigra:
- Dark due to melanin.
- Controls movement (linked to Parkinson’s disease).
- Tegmentum:
- Continuous with pons tegmentum.
- Contains important nuclei and tracts (Red Nucleus, Lemnisci).
🟣 3. Cranial Nerves in the Midbrain
📌 CN III (Oculomotor): exits ventrally.
📌 CN IV (Trochlear): exits dorsally (the only one).
🖇 Vestibular nuclei send fibers to CN 3, 4, 6, 7, 11, 12 → help with equilibrium reflexes.
🟠 4. Reflex Centers (Tectum Functions)
📌 Superior Colliculus:
- Center for visual reflexes (eye & head movement in response to light).
- Has 7 layers (Stratum Zonale → Stratum Album Profundum).
📌 Inferior Colliculus:
- Center for auditory reflexes (sound orientation).
- Receives fibers from Lateral Lemniscus.
🟣 5. Important Nuclei and Tracts
📌 Red Nucleus:
- In tegmentum (upper midbrain).
- Origin of Rubrospinal tract.
- Coordinates muscle tone and posture.
📌 Lemnisci (Ascending Pathways):
- Medial Lemniscus: from Gracile & Cuneate nuclei, carries fine touch & proprioception.
- Lateral Lemniscus: auditory pathway.
- Spinal Lemniscus: pain & temperature pathway.
🟠 6. Decussations (Crossings) in the Midbrain
🚨 Inferior Midbrain (at Inferior Colliculus):
- Decussation of Superior Cerebellar Peduncles → connects cerebellum to red nucleus.
🚨 Superior Midbrain (at Superior Colliculus):
- Dorsal Tegmental Decussation → from Tectospinal tract.
- Ventral Tegmental Decussation → from Rubrospinal tract.
🖇 Total brainstem decussations = about 5 (including medulla).
🟣 7. Clinical & Functional Highlights
📌 Substantia Nigra → degeneration causes Parkinsonism.
📌 Red Nucleus → involved in motor coordination.
📌 Superior Colliculus → visual tracking.
📌 Inferior Colliculus → auditory reflexes.
